service agreement

This form is provided to all clients

A downloadable client agreement I _________________________("client") agree to 45 minute Neurodivergent Services UK sessions with Athena St.Juste, MHC, with no minimum or maximum requirement. Client acknowledges and agrees as follows: All information discussed during sessions will be kept confidential. The Life Coach must report any verbalized wish by client to harm self or others. Client and Life Coach will meet virtually, or in person, as needed on an agreed regular basis. Client will make a Venmo, Paypal or check payment prior to the start of our session. Each virtual session is £80.00. In person sessions are on a sliding scale (£80-100). Client understands that payment will be due prior to the scheduled session time. Cancellations must be called in 24 hours in advance. Same day cancellations will result in same day cancellation charge of £80.00.
